What do I need for my first appointment?

If you plan on using insurance, you will need to request a prescription of services from your child’s primary care physician (Ex. "speech therapy evaluation and therapy if recommended")

 

Fax your child’s prescription for services directly to Baio Enterprises at 1-888-416-1854.

 

You will be emailed our intake form to be completed and returned before your first visit.  Please also e-mail your insurance card or stop by the office so we can make a copy.

 

Do you take my insurance?

 

Baio Enterprises is currently contracted with:

  • DDD (Department of Developmental Disabilities - AZ)

  • Blue Cross Blue Shield (AZ)

  • United Healthcare (AZ &CO)

  • TriCare (AZ & CO)

  • ESA  (Empowerment Scholarship Account - AZ)

  • Private Pay 

  • *Baio Enterprises also has a sister company, Baio Institute.  Baio Institute provides scholarships to those in need of services or therapy items.  The member must have a diagnosis of Autism Spectrum Disorders to qualify.

Since a patient’s financial responsibility varies depending on insurance, we recommend that you verify your coverage before scheduling.  If your insurance doesn’t cover therapy, please inquire about other payment options.
